We partner with multiple agencies throughout Indianapolis, but we don’t duplicate services. The agencies maintain the case management – and we do what we do best, provide the mentors.

Are you from a Partnering Agency and want to refer a mentee? Please complete the following forms:

• Part A to be completed by the case manager
• Part B to be completed by the mentee

Please remember that both forms are needed in order to enter the referral into our system.
